Dashboard Report: PATH

This article is intended for use by PATH grantees.  Street Outreach and Support Services Only program funded with PATH funding are required to complete PATH uploads, per the requirements of the local PATH funder.   This article will guide PATH grantees on the canned report available to prepare and export their PATH program data for upload/submission to the PATH funder.  

PATH

The PATH report is a dashboard (or "canned") report in HMIS.  This means that any agency user with Enter Data As rights to the PATH HMIS project can pull this report.  It is encouraged that PATH accessing users routinely review this report to correct any errors prior to the PATH report being pulled.

PATH report: running the report

  1. Set Enter Data As
  2. Select Reports on the left-hand menu to open the report dashboard
  3. Under Custom Reports, click on PATH
  4. Complete the report prompts
    • Provider Type
      • Provider: Confirm the project showing is the same as that set as the Enter Data As   OR
      • Reporting Group:  Search for the current reporting group for your PATH program. The reporting group will combine both the Street Outreach and the Service Only projects together.  THIS IS THE OPTION THAT SHOULD BE SELECTED FOR THE OFFICIAL FUNDER SUBMISSION!
    • Program Date Range: Manually enter the reporting period start date and the reporting period end date.  Typically, the start date is the beginning of the fiscal year (10/1/20XX) or grant year and the end date is the last day of the month for which the project will be submitting (i.e. If the submission will be sent in by May 9th, the data included will be through the month of April - 4/31/20XX)
  5. Click Build Report

PATH report: reviewing the report

The report will appear on the screen with 6 tables; each table will be labeled with a purple header.

  1. Persons served during this reporting period: this table provides an overview of the clients served during the reporting period.  This total includes only persons who have an active entry and have one of the following recorded during the reporting period: CLS, service transactions, date of engagement, or date of determination.   Each individual row within this table pulls the details differently.  Please reach out to the Helpdesk as needed to better understand how the details are pulled in.
  2. Services Provided: this table reports the number of individual clients who are have an active Entry/Exit into the PATH project & are marked as enrolled in PATH (date of status determination and PATH enrollment data elements are completed) who received PATH specific services during the time period, by PATH service type.  This table will not report the total number of services (i.e. A client with six recorded case management services in HMIS will only be counted once for case management type of service).  
  3. Referrals Provided: this table reports the number of individual clients who are have an active Entry/Exit into the PATH project & are marked as enrolled in PATH (date of status determination and PATH enrollment data elements are completed) who received PATH specific referrals during the time period, by PATH referral type.  This table also reports the number of individual clients whose outcome for the referral was marked as "Attained". 
  4. Outcomes: this table reviews outcomes for various different assessment data elements for those clients marked as enrolled in PATH.
  5. Destination at Exit: this table reports on only exited clients during the reporting period; it displays the location for which a client would be stay following exit from the PATH program.
  6. Demographics: this table provide basic demographic and housing history details of the clients served during the reporting period who have been marked as enrolled in PATH.

If the number in the cell is greater than 0, the number will appear blue and can be clicked.  Clicking the number will open a table that displays the client names and HMIS IDs associated with that data element and column.

 

PATH report: corrections

PATH grantees will want to review the report to confirm and correct the data during the reporting period. Below are some key items to review:

  1. Confirm the details from the Persons served during this reporting period table appear accurate for those clients served.
  2. Review the Services Provided and the Referrals Provided tables. These tables should reflect the number of clients for which a PATH service &/or referral was recorded. If the numbers are not aligned for the HMIS services or referrals recorded, a review of clients who are missing from the PATH report services &/or referral should be completed.  Those missing clients should then have their HMIS Service Transaction tab reviewed AND have their Date of Determination & PATH enrolled status confirmed as accurate within their Entry/Exit project row.PATH_Services.jpg
    • The above image shows were in the client record users should look to confirm that an HMIS Service &/or Referral were recorded.  Once it is confirmed the service or referral are recorded, the pencil should be clicked on to confirm that the PATH specific data elements are also recorded.PATH_Services_Data_Elements.jpg

    • The above image shows the two PATH specific data elements within the Entry/Exit project row assessments [intake, interim or exit] that is required to be completed for some data to pull into the PATH report
  3. Review the remaining tables to confirm there are no "Data Not Collected" rows with a count higher than "0".   With the exception of the Destination at Exit table, all remaining tables are only valid for clients who have been marked as PATH enrolled on their assessments.   Clients should become PATH enrolled at - or after - they have been marked with a Date of Engagement.   Date of Engagement does mean that all required data elements on the project start assessment have been filled out.

 

PATH report: Additional Feature

The PATH report does contain a Client Filter.  This filter allows for a specific client ID to be entered into the search field and highlighted throughout the report.   If that specific client ID appears associated with any of the row counts, the count cell will become highlighted.   Multiple client IDs can be searched at once by adding a comma between each number.  PATH report: Downloading for export

The report for PATH can be exported to a zip-file by clicking Download. This button is just below the reporting prompts.   The zip-file should not be opened as the whole file will be needed for the PATH submission to the funder.
