Recording Referrals in HMIS

This tip sheet was created as a basic step-by-step guide for recording referrals within HMIS.  

 

Recording HMIS Referrals (general)

  1. Set Enter Data As (should match the project for which the service was provided) 
  2. Search for client record with ClientPoint
  3. Set Back Date Mode (should match the date the service was provided)
  4. Click on Service Transactions tab
  5. On the Service Transaction Dashboard, click Add Referrals
  6. At the top of the Add Needs screen, select all clients in the household as of the date the service is provided.
  7. Select appropriate need Service Codes from Quicklist or look-up and select needs then click Add Terms
     
  8. Select the correct provider from the Provider Quicklist and click Add Provider or search and find correct provider in search features. 
  9. Ensure Selected Providers is correct.
  10.  Make sure Needs Referral Date in Referral Data is correct. You can leave everything else blank in that section unless your community has decided to use it(which is rare).
  11. ALWAYS click the boxes in the Referrals Referred to Provider section.


  12. Check Date of Need is correct in Need Data. Select Need Status(usually Identified) and Outcome( usually Not Met) in Selected Needs section.
  13. Click Save ALL.
  14. Confirm the referral shows up on the Referrals tab before moving on.

Note: If your "Quicklists" are not set up please contact the Data Center for help getting those set.
