This article is a guide for entering/updating County/Parish HMIS Data and entering Enrollment CoC.
Steps for adding County/Parish Data Entry in a New Project Entry
1. Use EDA and Backdate modes to enter the Data into the appropriate HMIS project on the Project Start Date
2. Search for Client
3. Navigate to their Entry/Exit tab & Click "Add Entry/Exit"
4. Make sure that the boxes next to each household member being enrolled are checked
5. Choose Enrollment Type
6. Click "Select County/Parish"
7. Click "Select County/Parish"
8. In the "Filter by State" section, select "North Carolina (NC)"
9. In the "Search by Name" section, search for the name of the County/Parish
10. Click on the green plus sign icon next to the name of the correct County/Parish
11. Click "Save & Continue"
Steps for Updating "County/Parish" in Open Enrollments
1. Set EDA and Backdate modes to match Project Start information
2. Search for Client
3. Navigate to their Entry/Exit tab
4. Click the Edit Pencil next to the Current Enrollment Project Start
5. Make sure that the boxes next to each household member being enrolled are checked
6. Click "Select County/Parish"
7. In the "Filter by State" section, select "North Carolina (NC)"
8. In the "Search by Name" section, search for the name of the County/Parish
9. Click on the green plus sign icon next to the name of the correct County/Parish
10. Click "Save & Continue"
Enrollment CoC Data Entry
Enrollment CoC is required to be collected for all adults in every enrollment and should reflect the Continuum of Care in which the HMIS project is located in.
Examples of Enrollment CoC and County/Parish Locations
Balance of State Continuum of Care
John is staying in a privately funded emergency shelter located in McDowell County, which is in the Balance of State CoC
- John's Enrollment CoC: NC-503 Balance of State
- John's County/Parish: McDowell County
Durham Continuum of Care
Makayla is in an SSVF Rapid Rehousing Project funded to operate in specific counties. The specific counties fall into NC-503 Balance of State CoC, NC-502 Durham CoC, and NC-513 Orange CoC. Makayla is housed within Durham County, which is in the Durham CoC
- Makayla's Enrollment CoC: NC-502 Durham CoC
- Makayla's County/Parish: Durham County
Orange Continuum of Care
David is in an ESG funded Street Outreach Project funded to operate with Orange CoC. The client is currently sleeping in a shed without utilities in Chapel Hill, which is in the Orange County CoC
