This report pulls basic information about months in HMIS service and disabling condition responses to support a possible Chronic Homelessness Determination status. This report should not be use exclusively for determinations but as an additional tool/support. This report only pulls in details from project types: Emergency Shelter and Street Outreach.

Setting Prompts
- EDA Provider: leave as -Default- (if running for a Back@Home project, select the specific project)
- Client Uid: enter the HMID ID for each client to be included in the report

This report has 5 tabs
Instructions
Provides basic description of the report and each tab.
Services by Month
This tab reflects recorded HMIS service transactions by emergency shelters &/or street outreach project types. Information is grouped by year per client. Each table reflects a calendar year for which service transactions were recorded in HMIS. Each row within the table represents a Service transaction provider, month the service was provided and the total number of days the service was recorded.
Admissions by Month
This tab reflects emergency shelter &/or street outreach project entry/exits for the client(s) over time. The table shows the calendar year the entry/exit occurred, the month the client exited (if applicable), project name, project start date, and project exit date (if applicable). The table also include the client length of stay (days), as well as, a count of HUD calendar months served.
Per HUD, a full calendar month is counted even if the client was served just one day within that calendar month. For example, Client A was served 2/28, 3/1, and 3/2. HUD would count 2 months: February (2/28) and March (3/1, 3/2).
Disability Data
This tab includes disabling conditions recorded as "Yes" by the ES or SO project at the time of entry for the client. Disabling conditions highlighted as green mean they meet HUD's Chronic Homelessness criteria for "disabling condition". Conditions not highlighted can be re-reviewed by the user to confirm a data element was not missing &/or inaccurate information was entered and needs to be corrected.
NOTE: For the disabling condition to appear on the report, disability data should have been recorded with the same Enter Data As mode as the project completing the project start.
Detail Data
This tab includes basic HMIS Project entry details for the client.
Requesting the Report
This report is a great documentation tool for establishing Chronic Homeless Determination, but you may not have the visibility to pull the report across CoCs or Regions. Email the Data Center Helpdesk if you are working to document Chronic Homeless status and want to check if other agencies have served your client.
