This report displays adult street outreach clients' enrollment details for a designated provider. The report focuses on (1) project enrollment details for adult street outreach clients' with an open, active project enrollment and a Current Living Situation (CLS) recorded most recently by the designated provider, (2) adult street outreach clients' with an open, active project enrollment who are missing a CLS recorded as the designated provider, (3)project enrollment details for adult street outreach clients' with an exit date to their project stay, and (4) a list current & exited adult client HMIS IDs along with project start/exit dates. This report requires that the clients' project enrollment provider matches the EDA provider used when recording the Current Living Situation!
This report is developed for Street Outreach Projects, however, it can be used by other project types for which CLS are recorded. It should just be noted that the Date of Engagement details would not be applicable to other project types.
NOTE: This report is available only to those HMIS users with an additional Advanced Reporting Tool license

- Enter Provider(s): select the specific HMIS street outreach project
- EDA Provider: select the same provider as entered above
- Enter Effective Date: the last day of recorded data to be included in the report + 1 additional day
- Report End Date (Plus one day): the last day of recorded data to be included in the report + 1 additional day *this likely will be the same as the effective date
- Report Start Date: the first day to be included in the report
Using the Report
This report has 5 tabs.
Summary
This tab includes details on the prompts selected to pull the report, a summary of totals from within the report, and a brief written description of the report and how to use it.
Reminder while reviewing each of the tabs listed below: if the CLS was recorded using Enter Data As mode for a project that is different that the project the client has a project entry (and the report prompt), the CLS recorded by that "other" project will NOT appear on this report! Example: Client has an project start date for the agency's ESG Street Outreach project. The most recent CLS recorded for the client was yesterday but was entered using EDA mode for the agency's private outreach project. This CLS would be excluded from the report.
Current Clients Overview
The top of this tab includes a written description of what clients are included in the table and details on what data elements to review &/or correct within the table. Each row on this tab is a unique project enrollment for adult clients; meaning if a client has a dual enrollment, the client will appear twice on this tab.
Errors to Identify on this tab:
- Any client appearing twice for the same project. Clients should only have one project stay at a time. Project enrollment dates should not overlap.
- Date of Engagement recorded prior to Project Start Date.
- Missing CLS Start, Information, or End Dates; these dates should all be the same date.
- Missing Current Living Situation housing status
- Missing Living Situation Verified By, which should list the specific project gathering the CLS
- Any CLS recorded over 90days. A current CLS should be recorded or the client should be exited as of the last date CLS &/or last assessment date, whichever is most current.
- Missing response to required to leave within 14 days from Current Living Situation housing status situation that is not literally homeless. Any client not in a place meant for habitation or shelter are required to have the follow-up question(s) completed.
- REVIEW ONLY (may not be an error) Any client enrolled more than 30days without Date of Engagement. This could be accurate but worth confirming

Clients with Missing CLS
The top of this tab includes a written description of what clients are included in the table. Every client on the table are those still enrolled in the project but do not have a CLS recorded by the project. To correct, each client should have at least CLS at project start date recorded by the provider (with the correct EDA mode!).
Exited Clients Overview
The top of this tab includes a written description of what clients are included in the table.

All SO Clients
The top of this tab includes a written description of what clients are included in the table. This tab should be used to verify the inclusion of each adult client for which the project had enrolled & served during the reporting period (both current and exited clients). Clients with duplicate project stays should be reviewed to confirm there are not overlapping Entry/Exit dates.
