B006 - Client Contact Information Report Guide

This report pulls Head of Households who has both a Client Contact recorded and an entry/exit into at least one project listed within the prompts, during the selected reporting period.   This report includes: HMIS ID, client full name, Client Contact details (numbers, emails, etc), Entry/Exit provider with dates, and last Current Living Situation recorded (location and verifying provider)

NOTE: This report is available only to those HMIS users with an additional Advanced Reporting Tool license

Recommended Export Format

Schedule to Microsoft Excel - Reports

 

Setting Prompts

  • EDA Provider:  (if CE Lead) select the Coordinated Entry project or (if any other) leave as Default
  • Enter effective date: Same date as the Report End Date prompt
  • OPTIONAL: Entry/Exit Provider(s): select desired Entry/Exit provider
  • OPTIONAL: Provider Group: Select the appropriate reporting group (such as the regional coordinated entry group, if running as CE Lead)
    • NOTE:  At least one of the prompts (Entry/Exit Providers or Provider Group) needs to be filled out!
  • Report End Date Plus 1 Day: Enter the last day of the reporting period and add one day (often the current date)
  • Report Start Date: Enter the first day of the reporting period

Client Contact List

This tab lists Heads of Household with contact information, Entry/Exit details, and most recent Current Living Situation (CLS).  Each row contains Head of Household name, listed phone numbers, social media handles, emails, other possible contact methods, and access to texting. The row also contains Entry/Exit details for which the client was served during the reporting period.   The CLS reflects the most recently recorded with provider verifying the client's current location at that time.   If a client's CLS was last recorded 90days or longer, the CLS details are highlighted.  If the client lacks a CLS, "No CLS recorded" will appear.

 

Additional Details

This tab lists the prompt details for which were entered to retrieve the report details.

 

REMINDER: When downloading or printing reports containing Personally Identifying Information (PII), extra security precautions should be taken. Be sure to delete downloaded files from your computer when they are no longer needed. Be sure to remove PII (names, date of birth, etc) before emailing or sharing the report with others!
