A010 - PATH Services & Referrals Report Guide

This report is developed for PATH programs only!

This report displays (1) clients with an entry into the PATH project, as well as exit details for those exited, (2) details and data quality related to PATH determination status, (3) identifification of which PATH clients are required to have PATH specific services & referrals required following PATH enrollment, (4) PATH service and referral details, and (5) any service note recorded.

 

It is encouraged this report is run in conjunction with the "A009 - PATH Details Data Quality" report.

NOTE: This report is available only to those HMIS users with an additional Advanced Reporting Tool license

Setting Prompts

    • Enter value(s) for Provider: select the specific HMIS PATH project
    • EDA Provider: select the same project as listed under the prompt above
    • Reporting End Date: the last day of recorded data to be included in the report 
    • Reporting Start Date: the first day to be included in the report
    • Enter effective date: enter the same date used for "Reporting End Date" prompt

Using the Report

This report has 7 tabs.

Summary

This tab includes a summary of counts for the designated provider. Counts included are:

  • total clients with an project entry
  • total clients with a service recorded
  • total number of services created
  • total clients with a referral recorded
  • total number of referrals created, total clients with "yes" recorded for PATH enrollment
  • total count of clients with "Yes" recorded for PATH enrollment and a service recorded
  • total count of clients with "Yes" recorded for PATH enrollment without a service recorded
  • total count of clients with "Yes" recorded for PATH enrollment and a referral recorded
  • total count of clients with "Yes" recorded for PATH enrollment without a referral recorded.

 

To review: confirm the total clients with a project entry/exit appears accurate for the number of clients served/contacted during the reporting period.  Confirm the number of clients marked as PATH enrolled appears accurate.   The total clients with PATH enrollment should equal the "total PATH enrolled clients with services" + "total PATH enrolled clients without services".  If the numbers do not add up, you will want to check for duplicate/dual enrollments (the same client served more than once during the reporting period).    The total clients with PATH enrollment should also equal the "total PATH enrolled clients with referrals" + "total PATH enrolled clients without referrals".  If the numbers do not add up, you will want to check for duplicate/dual enrollments (the same client served more than once during the reporting period). 

Clients with Entry/Exit

The top of this tab includes a written description of what clients are included in the table and details on what data elements to review &/or correct within the table.   Each row on this tab is a unique project enrollment for adult clients; meaning if a client has a dual enrollment, the client will appear twice on this tab.

Errors to Identify on this tab:

  1. Clients identified as not PATH eligible but are without an exit from the project
  2. Exited clients who are missing the PATH date of determinatin and PATH enrollment details
  3. Clients with a PATH enrollment response but missing date of determination.
  4. Clients with a date of determination that is prior to project start or after project exit.
  5. REVIEW ONLY (may not be an error):  Any client without a PATH status determination and PATH enrollment but has been in the project more than 3 months.
  6. REVIEW ONLY: Any client appearing twice for the same project. Clients should only have one project stay at a time. Project enrollment dates should not overlap.

Exited Clients with Destination

The top of this tab includes a written description of what clients are included in the table and what to review for correction.  Every client on the table are those who have an PATH project exit.  This tab includes the exit destinations of those clients.

Errors to Identify on this tab:

  1. Any client with an exit destination of "Data Not Collected".  Refer to Exit Destination Guide for guidance on selecting appropriate destinations.
  2. Clients with missing date of determination &/or PATH enrollment.  All exited clients are quired to have these items filled out by exit.  If unknown, the date of determination should equal the exit date and the PATH enrollment set to "No" with the required reason not enrolled completed.  Note: If both the date of determination and PATH enrollment are missing, the error will appear as yellow with red writing.  If just the date of determination is missing, the error will appear as green.
  3. Clients with date of determination prior to project start date or after project exit date.

Service Details

The top of this tab includes a written description of what clients are included in the table and what to review for correction.  Clients on this table are limited to only those who are marked as "yes" for PATH enrollment.  Each row is a service recorded by the provider. If the service was not entered into HMIS using the EDA mode for the PATH provider selected within the reporting prompt, the service will not appear on this report.

To review:  confirm all PATH enrolled clients appear on this table if a service was provided.  If a client is missing, check if the PATH date of determination and PATH enrollment fields have been completed &/or confirm a service was recorded by the specific PATH provider.   Additionally, check any services on this report which are missing the "PATH Service Type".   If this service was funded through PATH efforts, the HMIS service must be edited to fill in the missing PATH specific data element required.

Referral Details

The top of this tab includes a written description of what clients are included in the table and what to review for correction.  Clients on this table are limited to only those who are marked as "yes" for PATH enrollment.  Each row is a referral recorded by the provider. If the referral was not entered into HMIS using the EDA mode for the PATH provider selected within the reporting prompt, the referral will not appear on this report.

 

To review:  confirm all PATH enrolled clients appear on this table if a referral was provided.  If a client is missing, check if the PATH date of determination and PATH enrollment fields have been completed &/or confirm a referral was recorded by the specific PATH provider.   Additionally, check any referrals on this report which are missing the "PATH Referral Type" or "PATH Referral Outcome".   If this referral was funded through PATH efforts, the HMIS referral must be edited to fill in the missing PATH specific data elements required.

The image above shows BOTH specific PATH required data elements within the HMIS referral which must be selected to fulfill the PATH requirement. Both of these items are required.

Service Notes

This tab displays any notes recorded within the HMIS service or referral.   The notes are grouped together by client ID and in date descending order.  Notes recorded on services/referrals are not a requirement for the PATH program but some agencies may internally set this as best practice. Please check in with the Agency Admin at your agency to confirm internal practices related to notes.
