This report is developed for PATH programs only!
This report displays (1) adult clients' PATH specific enrollment details for an entry&/or exit into the designated provider during the reporting period, (2) adult PATH clients with an open, active project enrollment and their most recent Current Living Situation (CLS) information recorded as the desingated provider, and (3) adult PATH clients who are both still actively enrolled and those who exited who are missing a CLS recorded as the designated provider. This report requires that the clients' project enrollment provider matches the EDA provider used when recording the Current Living Situation!
It is encouraged this report is run in conjunction with the "A010 - PATH Services & Referrals" report.
NOTE: This report is available only to those HMIS users with an additional Advanced Reporting Tool license

- EDA Provider: select the specific HMIS PATH project
- Select Provider(s): select the same project as listed under the prompt above
- Report Start Date: the first day to be included in the report
- Report End Date (Plus one day): the last day of recorded data to be included in the report + 1 additional day
Using the Report
This report has 5 tabs.
Tab A- Summary
This tab includes a summary of counts for the designated provider. The counts include:
- unduplicated client count
- number of entries [this could include duplicate client counts if a client was entered multiple times into the project during a reporting period]
- total number of clients with a date of engagement,
- the number a number of exits [this could include duplicate client counts if a client was entered & exited multiple times by the project during the reporting period.
Reminder while reviewing each of the tabs listed below: if the CLS was recorded using Enter Data As mode for a project that is different that the project the client has a project entry (and the report prompt), the CLS recorded by that "other" project will NOT appear on this report! Example: Client has an project start date for the agency's PATH Street Outreach project. The most recent CLS recorded for the client was yesterday but was entered using EDA mode for the agency's ESG street outreach project. This CLS would be excluded from the report.
Tab B - Details
The top of this tab includes a written description of what clients are included in the table and details on what data elements to review &/or correct within the table. Each row on this tab is a unique project enrollment for adult clients; meaning if a client has a dual enrollment, the client will appear twice on this tab.
Errors to Identify on this tab:
- Date of Engagement recorded prior to Project Start Date or after the Project Exit Date.
- Exited clients with a missing Date of Status Determination &/or PATH enrollment Status. These data elements are required by the time a client exits the PATH project.
- Clients marked as "No" for PATH enrollment but are missing a Project Exit Date
- Clients marked as "No" for PATH enrollment but are missing the follow up "if no..." data element.
- Client is missing a SOAR connection response. This is required at Project Start & can be updated throughout the project stay.
- REVIEW ONLY (may not be an error) Any client in the project more than 90days without Date of Engagement. This could be accurate but worth confirming
- REVIEW ONLY (may not be an error) Any client without a PATH status determination and PATH enrollment but has been in the project more than 90days.
- REVIEW ONLY: Any client appearing twice for the same project. Clients should only have one project stay at a time. Project enrollment dates should not overlap.

Tab C - CLS
The top of this tab includes a written description of what clients are included in the table and what to review for correction. Every client on the table are those with an entry in the project and have a CLS recorded by the project. Only the most recent CLS will appear on the report.
Errors to Identify on this tab:
- Any client wiith a Current Living Situation which is 90days or older.
- Any client with a missing CLS Start &/or Information date.
- Any client with a missing Living Situation Verified by
- Any clients in a non-literally homeless living situation without details on if the client is having to leave the situation within 14days
- REVIEW ONLY (may not be an error): clients in a non-literally homeless living situation recorded more than 30 days ago

Tab D - Missing CLS
The top of this tab includes a written description of what clients are included in the table and what to review for correction. Every client on the table are without a CLS recorded by the project. To correct, each client should have at least a CLS at project start date recorded by the provider (with the correct EDA mode!). Note: if a client has more than one project entry during the reporting period, the client may appear twice on this table.
Tab E - Additional Information
This tab includes a table displaying the prompts entered in order to pull the details included within the report.
